Scheduling an Appointment
Calls to make appointments can be made beginning at 8:15 a.m. We reserve 8:15 a.m. until 9:00 a.m. for calls about urgent same day appointments. During this time call (909) 629-5067 and choose option 3. This same number can be used for routine appointment calls between 9:00 a.m. and 5:00 p.m. After 9 a.m. to get an urgent same day appointment use option.

We are not set up to see walk-in appointments. Please call before coming to the office so we can reduce the wait time for all of our patients.

Providing quality medical care for children and adolescents
We encourage you to use the phone during office hours. Many problems may be solved with advice over the phone, saving you the time and cost of a visit. We are happy to provide this service, but it is against the policy of this office to treat by phone when there is a more serious illness or a patient who has never been seen in our office. Whenever the office is closed, the doctor on call may be reached at home by calling our office number, (909) 629-5067 and pressing 9. Remember, however, that we also require sleep to function properly. Please call at any hour if your child is truly ill but save routine calls for regular office hours.
Immunization Records and Forms
There is a 5-7 day turnaround time for these forms to be filled out unless you bring them with you at the time of your well check appointment.
Well Checks
We encourage you to make summer well check appointments for camp, school, or sports physicals early in the season. We are very busy with these appointments in August and early September and may not be able to accommodate you on short notice during this time of year. Please bring your child's immunization record to all well check appointments. If your child is sick, please keep your well appointment.
Payment Policy
It is our office policy that all payments are due at the time of service. This would include any portion of the service not covered by insurance. Payment can be made by cash, check, and credit/debit card with a Visa, Discover, American Express, or MasterCard logo. If you are not able to make payment at the time of service your appointment may be rescheduled. (Exceptions will be made for acutely ill children.) All copayments are due at the time of service. A finance charge will be assessed for all missed copays.
No Shows and Late Shows
You must bring a valid insurance card and your co-payment (if applicable) to each appointment. Also, please bring your child's immunization record to any well check appointment. If you have new insurance and have not yet received your insurance card, you will need the following information: insurance company name; claim address; co-payment amount; and policy holder name, policy number, group number, and suffix.
If you will not be able to keep an appointment, please call our office to cancel as far in advance as possible so that we can see another patient in that time slot. We will not be able to continue to care for your child if you repeatedly fail to keep appointments. If you are more than 15 minutes late for an appointment, we may need to reschedule you.

  • Friday 8:30 a.m. - 5:00 p.m.; closed for lunch 
  • Urgent Saturday morning appointments
  • Late afternoon/early evening appointments available Monday - Thursday.
